Reason:Admitted for elective Hartmann's procedure. Surgery performed March 9 by Dr. Williams — uncomplicated. Post-operative recovery progressed as expected. Stoma functioning on day 2. Pain well controlled by discharge. Oral intake re-established. Ambulating independently.
DISCHARGE MEDICATIONS
Medications:Oxycodone 5mg Q4-6H PRN (pain) · Paracetamol 1g QID · Coloxyl 120mg BD (stool softener) · Omeprazole 20mg daily · Enoxaparin 40mg SC daily x 28 days (DVT prophylaxis)
FOLLOW-UP INSTRUCTIONS
Appointments:Surgical review — Dr. Williams clinic March 19, 2026 at 10:00am · Stoma nurse outpatient — March 14, 2026 · Oncology consult — March 26, 2026 (re: adjuvant chemotherapy plan)
Wound care:Dry dressing to midline wound — change every 2 days or if soiled. No showering until wound reviewed at surgical review. Stoma pouch change demonstrated — patient verbalized understanding.
Return if:Fever above 38°C · Wound redness, swelling or discharge · Stoma not functioning for more than 48 hours · Severe or worsening pain · Any concerns
PATIENT EDUCATION PROVIDED
Education:Stoma care — patient and wife demonstrated competency. DVT prevention — compression stockings and daily enoxaparin explained. Diet — soft diet for 2 weeks, avoid high-fibre initially. Activity — no heavy lifting for 6 weeks.
