Medical Diagnosis:Stage III breast cancer — post chemotherapy Day 3
NURSING DIAGNOSIS
Problem:Risk for infection related to chemotherapy-induced neutropenia as evidenced by WBC 1.8 x10⁹/L
GOALS / EXPECTED OUTCOMES
Short-term:Patient will remain afebrile and show no signs of infection within 48 hours
Long-term:Patient will demonstrate understanding of neutropenic precautions prior to discharge
NURSING INTERVENTIONS
Interventions:Strict hand hygiene enforced for all staff and visitors. Neutropenic diet commenced. Observations 4-hourly including temperature. Protective isolation maintained. IV access site assessed every shift. Patient and family education re: infection signs completed.
EVALUATION
Outcome:Patient remains afebrile at 36.8°C. No signs of localised infection. Patient verbally confirmed understanding of neutropenic precautions. Plan to continue and reassess at 24 hours.
