PMHx: T2DM, HTN, Ex-smoker. Medications: Metformin, Perindopril, IV Ceftriaxone. Allergies: Penicillin (rash).
A — ASSESSMENT
Deteriorating — MO notified at 18:30. Obs every 30 mins. NEWS2 score 5. Increased respiratory effort noted.
R — RECOMMENDATION
MO review pending. Escalate immediately if SpO2 drops below 90% or RR exceeds 28. Repeat bloods at 22:00.
